All entrants must submit by December 3rd, 2009 to Miss Martin:
-Checklist for Adult Sponsor (1)
-Student Checklist (1A)
-Research Plan
-Approval Form (1B)
*Teams must also complete Student Checklist (1A) –TEAM

CCSRC Official Entry Form
This form is not needed at this time. It is only available in paper form right now. It will need to be completed by students moving on to the CCSRC.

Special Reminders!
Please remember that any projects that includes human subjects, vertebrate animals, hazardous biological agents and/or chemicals will NOT be approved.

Approval will be granted on a rotating basis, so feel free to hand forms in early!

Experiments CANNOT begin until after project has been approved by Susan Martin.

Team projects cannot convert to individuals, nor individual projects to teams.
